2.0: Introduction

Root Cause Analysis (RCA) Using Tripod Beta Method is a two-day program designed to provide participants with the necessary skills and knowledge to effectively implement the TCA using Tripod Beta methodology. The Tripod beta method is based on the Human Error theory. This theory states that incidents happen when people make errors and fail to keep the barriers functional or in place. These errors are Immediate Causes. The RCA Tripod Beta is a structured approach that focuses on identifying the root causes of problems and implementing corrective actions to prevent recurrence.
